Wow, I took a double, then triple take of that photo of Keith with the sculls. The photo is taken at Warhol's factory. My friend and bandmate, Walter Steding, who was Warhol's painting assistant jammed with Keith that same day. We only have one photo from that day and he has never seen the one that was posted in this thread. I just spoke to Walter and the photographer was Chris Makos.
