As wrote on the jacket ... this product has not been autorised or approved by the RS or their management (so I hope is OK to share it here)

Short manual to rebuilt the DVD :
Step 1 : Create an Artwork folder and a VIDEO_TS folder.
Step 2 : unzip the zip folder.
Step 3 : Put the artwork and info files into the Artwork folder.
Step 4 : Put all the other files in the VIDEO_TS folder.( here the files are in the two parts of every DVD)
Step 5 : use a DVD burner to burn the files from the VIDEO_TS folder.

Hey StonyRoad (good job, BTW) or anybody else, can you post a good scan of the spine area that doesn't have the reflected light effects up and down the spine?

Sometimes holding the cover down on a flatbed scanner can overcome the effect.

Otherwise, can anybody point to any links that have a scan of the full cover? I haven't found anything else so far.

Yes I realise now that the scan is not great... but I'm now the country so I cannot re-do a scan.
The cover is complete (I added also the inlay and the label in the 'covers' file)
