bob dylan accidentally pushing his 'guest' Carole King off the stage in Dublin and breaking her leg.
Zappa breaking his leg and having serious trauma to his neck and back after being pushed offstage and into the orchestra pit at the London rainbow in 1972. he was wheelchair bound for ages
keith Moon collapsing over his drumkit in the mid 70s..they got a guy from the audience onstage to drum
wasnt there a guy in a metal group shot dead onstage in the US a couple of years ago? Doesnt get much more serious than that.
I'm trying to think of accidents happened to The Stones. Keith was nearly electrocuted somewhere around 1965. Apart from that nothing comes to my mind for the moment. Well, unless you count Altamont, but it wasn't ONstage as a matter of fact.
well, if you don't count fans dragging them off the stage, throwing things at them causing cuts that required stitches, destroying the equipment and/or trashing the venues, or the time when a stage collapsed, or the time Bill fell off the stage ... i guess the Stones haven't had *too* many accidents. :E

Wyman fell off the back of the stage at the end of the show in St Paul in 1978 and damaged his hand, I think.
Keith went on his arse a couple of times in 1976 (and 2007 for that matter)
During the infamous Blackpool riot in 1964, Brian got thrown offstage. A woman I used to work with told me about it as she was there. He landed right in front of her.

Green Day was on David Letterman a couple of years back and after their song, the drummer stands up and takes a header off of the front of his platform. I loved Dave's reaction. I occassionaly search youtube for a clip of it but have yet to find it.
Fay Waybill of the Tubes falling of his platforms and breaking his ankle, teh rest of the tour was cancelled, including the 14night run at hammersmith odeon. one of which i had tickets for.
Shane MaGowan unable to finsih a set at the 100 club after taking a beating from some squadies the same day. cush from the Men They Couldn't Hang franticly learning some chords in the dressing room so he could come on as a late sub. he came on after about 20mins. the gig ? well it was memorable.

I'm pretty sure the bassplayer of Nirvana was knocked unconscious after throwing his bass high into the air, and unable to catch it, the bass landed on his head.
I think it was on some award show, so it was filmed
Back in the early 80's I saw the Who at Sun Devil Stadium on the campus of Arizona State University. John Cougar (not Melloncamp yet) was one of the opening acts. Someone in the audience threw something at him (a rock, or bottle, don't remember exactly) and hit him in the head. He immediately left the stage and after about 15 or 20 minutes, because of a supportive crowd, came back in did the rest of his set wearing a football helmut over his head.

Yeah what a bozo, i remember that. It was the MTV awards, he didn't pass out though he stumbled across the stage. Here's a link
Wasn't looking too good, but I was feeling real well.
I read that the Who were playing a college gig in late 1970 and were using art tables as a stage. Pete went to do one of his jumps and fell through the table. According to the report, he kept on playing standing in the table. the story: [www.thewholive.de]
