Anyone know where this term originated from? And when?
Like when Ya-Ya's opens with The Greatest Rock & Roll Band In The World, the Rolling Stones!

Who coined this phrase?

I am not 100% but thought it was Sam Cutler when he introduced them in '69. I know it's fitting,that's for sure.

yep: Cutler first called them that at the Hyde Park gig.

The Greatest Rock'n Roll Band?
On the nights of early 70's when the Stones were off, the greatest were surely Mad Dogs & Englishmen. I can not think of another group that put the whole Exile/ Gospel/ Blues spirit on stage better. Rock'n roll at it's finest.
Bobby Keys and Jim Price knew how to pick them.

Actually Delaney and Bonnie knew how to pick them as the Mad Dogs ensemble was an offshoot of Delaney & Bonnie and Friends. (which included Bobby and Jim, among other fine picks and pickers)

What a great band though. Or the whole feel. I love watching those hippie girls doing the BU vocals. Chris Stainton always has a zonked out girl by his side gyrating.
What is great about watching the Cocker film footage is that everyone is laughing, including audience, because everyone knows that this is it. I have only seen that at Stones shows otherwise.
