With many thanks to a friend, here's the version from the "Sweet Black Angels" label.
This label's productions are only sold in a quantity of 50 items.

Do not ask me what the source is or how my friend got it.
Unfortunately, the artwork that comes with this upload only has the front artwork.
The rest are screenshots with info.
