I would have said Trent Reznor's 'Hurt' beforehand but Johnny Cash took it to a whole new level.
Most of Scott Walker's stuff from 'Tilt' and 'The Drift' is just too depressingly personal albeit brilliant. Similar tale with a lot of Roger Water's solo output.

A few years ago, I went to one of those Beatlefests gatherings at a Meadowlands hotel. I took my son, then six years old. It was fun. They had a battle of the bands one night. Each act was allowed one Beatles song or solo-Beatles song.
To my surprise, some guy did "Mother"... complete with painful wailing near the song's end.
