Wrong. It wasn't released in Europe / UK, until 1985 (as two seperate releases).
The initial UK release, or the one advertised as a double album, was in 1990.

As far as I know, the photo was taken especially for the compilation, thus a fresh new photo.
"Hot Rocks" was the first part of the original settlement with Klein / ABKCO and it was with The Stones' own blessing it was released. It was a different matter in Europe, where Decca released substandard compilations, without any Stones involvement.
The ABKCO / Stones agreement was that they could release 2 compilations + one with outtakes. "Hot Rocks" was the first, the second was to be "Necrophilia", which was then cancelled and instead replaced with "More Hot Rocks".
The Stones (Bill Wyman and Charlie Watts), oversaw a compilation of outtakes
(Black Box) in hope to gain a little control, however ABKCO chose to make a combination of the scrapped "Necrophilia" and "Black Box", thus "Metamorphosis" was born.

In all likelihood, since Bill never signed the Virgin contract, you can't include him on an album cover without coming to an agreement. Why bother when he can be photo-shopped out? What's the worst that will happen - a bunch of obsessive compulsive fans will discuss it on a message board?

Except for 2 tracks, "Godzi" and "Panama Powder Room", all of it has been released by now on other compilations: [www.50thirdand3rd.com]

If a band release a bunch of songs, and one or more of the original members isnt in the band anymore then these members should be included in the photo shoot if even one of the songs on the album was of their time.
Different case if the albums live.
