Keith had already been a backing musician for Billy on his Apple debut album. (And Taylor backed him in '73.)

Not on the '75 and '76 tours; it was de rigeur at every concert (afaik). Many of the reviews of the '75 US shows cited Billy's set as a highlight.

Oh please, they knew it was being filmed for a television broadcast. The Stones just aren't as uptight as some of their fans.
